We are now taking appointments for COVID-19 Vaccinations. 

Currently, we are offering the Pfizer and Moderna Vaccines both of which require 2 injections. Pfizer is now approved for ages 12 and up.  Moderna is approved for ages 18 and up.

How to prepare for your appointment

​

• Arrive on time and no more than 5 minutes earlier than your scheduled appointment time. We have limited space for parking and patients.  

• Bring a photo ID and if you have your insurance card. 

• If you do not have insurance, you can still get the vaccine. You will not be asked to pay or be billed for the COVID vaccine.   

 

What to expect when you arrive​

 

• ​Fill out registration paperwork

• Make an appointment for 2nd dose of vaccine prior to leaving (21 days for Pfizer, 28 days for Moderna)

• You will receive a card with vaccination information. We recommend you make a copy or take a photo to save the information. We do not issue replacement cards.

• Receive your vaccine and return to your car and wait for 15 or in some cases 30 minutes before driving away.
